We are thrilled to announce an exciting new opportunity to celebrate the achievements of our student-athletes: the inaugural IADA All-State Recognition Awards! The IADA All-State Recognition Award is independent of the IHSA All-State recognition.

Each IADA member athletic director may nominate one female and one male student-athlete who exemplifies excellence in all areas. Here are the key eligibility requirements:

  • The nominee must be a second-semester senior.
  • The student must have participated in at least two IHSA sports and/or activities during each of the last two years.

A sample PDF nomination form is linked here so that you may review the content of the online nomination form as you contemplate which male and female athlete to nominate from your school.

Only online nominations will be considered. One male and one female nominee will be named to the all-state team from each of the 21 Districts. Additionally, one male and one female nominee from each District will be named to the honorable mention team.

To nominate your outstanding student-athletes, please log in to your Final Forms portal, where you’ll find the All-State Nomination Form. An instructional video is linked here to guide you through accessing the nomination form. If you have administrative access in Final Forms, then you will log in to your portal and then click “Manage” next to your name and then scroll down to the Awards section and click “Manage” to access the nomination form.

Online Application Deadline: February 10, 2025

Please note: Incomplete or late applications will not be considered, and eligible students will not receive a certificate of recognition if their application is not submitted on time.
